Types of Providers
At Northwest Transitions, an interdisciplinary team works together to create a care plan tailored to each patient. During your stay, you may interact with:
Rehab Nurses - Rehab nurses are specially trained in rehabilitation care and techniques. Your nurse will provide care 24 hours a day during your stay. They treat the whole patient and family through education, active listening and participation in family and team meetings.
Rehab Physicians - Physicians are specially trained in rehabilitation and recovery. They work with each patient, overseeing the direction of their care, with a focus on physical and medical recovery.
Physical Therapists - Patients work with physical therapists to improve their strength, endurance and balance. They focus on functional independence with walking, sitting and standing. They also help train family members to assist their loved ones once they return home.
Occupational Therapists - Occupational therapists focus on activities of daily living, like dressing, bathing and using the restroom. They also help with re-learning household tasks, like cooking and cleaning.
Speech Therapists - Speech therapists work with patients to improve the expression of their wants and needs, management of home care tasks, memory, social skills and swallowing. They may focus on cognition, communication or swallowing to help patients meet their goals.
Case Managers and Social Workers - The team of social workers and case managers work with patients and their support network to ensure a safe transition from inpatient rehab to home, or to the next level of care. They work with patients and their families to make sure patients have a support structure in place and the resources needed to succeed at home, in the community or wherever their next destination may be.
