The admissions desk is located at the main hospital admitting department, located directly east of the ER entrance, easily accessible from the hospital parking garage. All patients who have a surgical procedure scheduled that involves an overnight stay should register here.

Outpatient Surgery: Patients who have an outpatient procedure scheduled and are going home the same day should use the surgery department entrance, located on the east side of the hospital. The east parking lot provides the most direct access.

OB/GYN Patients: The admissions department for the Women's Center is located at the front entrance to the Women's Center at 1920 W. Rudasill Road, south of the main hospital, on the left.

Women's Imaging: Patients who have a diagnostic test (such as a mammogram or bone density test) should check in at Northwest Women's Imaging, located adjacent to the Women's Center, at 6060 N. Fountain Plaza Drive.

Diagnostic/Pre-Surgical Tests: For patients who require tests before surgery, the pre-procedural screening office is located at 1980 W. Hospital Drive, Suite 111, in the building adjacent and to the east of the ER/outpatient entrance to the hospital.

Remember to bring:

• All current health insurance information, including your insurance card
• A photo ID
• Physician orders and prescriptions for any tests you will receive
• Referrals from your primary care physician (if your insurer requires them)
• A payment method for deductibles and co-payments (financing is also available)
• Your advance directive, if you have one
• Medicare/Medicaid card
• Social security number/card for yourself and the policy-holder, if you are covered under a family member's policy
• Driver's license/photo ID card for yourself and the primary insurance holder
• Emergency contact information
• A list of current medications including prescriptions, over-the-counter drugs, vitamins and herbal supplements you are taking
