Neurologic Rehabilitation
The Northwest Transitions rehabilitation team provides services for people living with brain injury due to accidents, surgeries or other neurological events. Our team of nurses, physicians and therapists specializes in neurologic rehabilitation, but is dedicated to treating the whole person. Speech therapy teaches the patient and caregivers strategies to manage changes in memory and attention. Physical and occupational therapy are designed to improve strength, balance and coordination for a higher level of day-to-day independence. Social workers and case managers help identify community resources to support a safe transition back home.
In addition, we care for patients living with Parkinson’s disease, multiple sclerosis, myasthenia gravis, Guillain-Barre syndrome, ALS and other neurological conditions. Our team strives to help patients achieve better strength, balance and coordination, and partners with patients and caregivers to identify the equipment, community resources, education and level of assistance which may be necessary for a successful transition home. Our physicians will adjust and initiate medications to manage the condition in the best way possible.
Our team includes therapists with special training in stroke recovery. Their goal is to create an individualized treatment plan which promotes strength, balance and the recovery of self-care and mobility skills. Patients who have experienced a stroke may also need specialized services designed to help with swallowing, speech, communication and cognitive difficulties. Nurses and physicians monitor patient vitals and offer stroke prevention education, while physicians adjust medications as indicated to manage stroke risk factors. Social workers and case managers identify equipment and support services to promote a successful transition home.
